Workers Comp Report Quick Training

Keep This in Mind

  • Take care of an emergency and seek medical help immediately.
  • In non life-threatening injury or illness, go to nearest Intermountain Workmed.
  • Fill out Worker's Compensation First Report of Injury Form as soon as possible and return it to Human Resources.
  • In case of emergency or serious injury call:

  • Kirt Poulsen (Safety and Industrial Hygiene Manager, 797-3507)
  • Steve Bilboa (Environmental Health and Safety Director, 797-2763), or
  • Environmental Health and Safety Office, 797-2892
  • What to do:

  • Take care of the injured employee. Provide first aid and if required take him/her to Intermountain WorkMed.
  • Report all accidents (either by manager or employee) as soon as possible of their occurrence to Kristie Larsen (Worker's Compensation Administrator, Office of Human Resources) at 797-0197.
  • Fill out Worker's Compensation First Report of Injury Form and send it to Human Resources as soon as possible.
  • If the injury is not serious, above is sufficient.
  • If the accident results in death or is very serious and/or life threatening according to a reasonable person, call 911 and report as soon as possible to:
    • Kirt Poulsen (Safety and Industrial Hygiene Manager, 797-3507)
    • Steve Bilboa (Environmental Health and Safety Director, 797-2763)
    • Environmental Health and Safety Office, 797-2892

  • Note: Secure the accident area and do not allow potential evidence to be disturbed by you or others.
  • If the accident occurs during off hours or over the weekend, you must immediately report to OSHA by calling 801-530-6910 and to Environmental Health and Safety by calling 435-797-2892 at the start of the next normal business day.

NOTE: In rare cases ex. violence, you may also need to report it to the USU Police Department at 797-1939.
